JOB TITLE:                        Outreach Therapist

EMPLOYMENT TYPE:       Full-time, 40 hours/week

SCHEDULED HOURS:       Monday-Friday, 8:30am-5pm, one week night 12pm-8:30pm

PROGRAM/LOCATION:   Assertive Community Treatment Team (ACTT); Manchester, CT

*Community-based travel, predominately within a 15-mile radius. Company vehicles readily accessible for use!*

Assertive Community Treatment Team (ACTT) serves adults 18 years of age and older.  ACTT provides intensive community-based treatment, support, and rehabilitation to individuals who have a serious persistent mental illness (SPMI) or a dual diagnosis of SPMI and a Substance Use Disorder and be determined as requiring a high level of service needs.

 Duties & Responsibilities

  • Conducts Clinical and Risk Assessments, including: LOCUS, Co-occurring Screens, DLA-20.
  • Collaborates with individuals to create individualized, person centered treatment plans and documents clients progress during treatment.
  • Provides individual and group therapy.
  • Use Motivational Interventions to assist clients in meeting their goals and moving along their paths to recovery, utilizing stage wise interventions
  • Provide psycho-educational rehabilitation services group and individual for recovery from psychiatric and substance abuse disorders
  • Support individual with linking and engaging with natural (family, friends) or community supports
  • Develop crisis plans for clients and work with other staff to respond to crisis situations as needed
  • Provide skills teaching, such as assisting clients in obtaining/ maintaining housing and utilities; learning how to use transportation services, planning and preparing nutritional meals, engage and communicate with family and friends, be safe in the community, care for children and other family members, and solve everyday problems that arise.
  • Provide intensive skill building interventions and support for self-management, self-advocacy, health and well-being, relapse prevention, and using community resources
  • Participate in family education and support activities
  • Maintain documentation in compliance with agency and program standards.
  • Participates in staff meetings, supervision, trainings and other meetings as assigned.
  • Additional duties as required.

QUALIFICATIONS:

Education: Master’s of Social Work.

Experience: 2+ years working with identified populations. Preferred: Experience working with families, providing family therapy; individual and group.

Licensure/Certification/Registration:  LCSW preferred; LMSW required. Valid Driver’s License.

COMPENSATION*:

$61,252.26/yr minimum, and up commensurate upon experience

*Actual rates are determined at the time of offer and are based on relevant experience, as well as, adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)
