Clinical Pharmacist – Acute Care Hospital

Facility Type: Acute Care Hospital Pharmacy (245 Beds)

Must Have
  • Active Texas Pharmacist License
  • Minimum 2 years of Hospital Pharmacy experience
  • Completion of a 20-hour ACPE-accredited Sterile Compounding Course (or equivalent documentation)
  • Experience with medication order verification and entry
  • Experience performing final medication dispensing checks
  • Knowledge of sterile compounding practices
  • Ability to supervise pharmacy technicians
  • Strong clinical judgment and medication safety knowledge
Preferred
  • Meditech experience
  • Decentralized Pharmacy experience
  • Vancomycin dosing adjustments
  • Renal dosing adjustments
  • Experience in acute care hospital pharmacy
  • Experience with clinical pharmacy interventions
Schedule
  • Day Shift: 7:00 AM – 7:00 PM
  • 12-Hour Shifts
  • Every Other Weekend Required (7-on/7-off Rotation)

  • Verify and enter medication orders accurately
  • Perform final verification of medication dispensing
  • Supervise pharmacy technicians during medication preparation and distribution
  • Review override reports and medication safety reports
  • Prepare and verify sterile compounded medications
  • Perform clinical pharmacy interventions and medication therapy management
  • Adjust vancomycin and renal dosing when appropriate
  • Ensure compliance with hospital, state, and federal pharmacy regulations
  • Collaborate with physicians, nurses, and the interdisciplinary healthcare team
  • Document clinical activities and medication interventions in Meditech
