Job description

Chief Operating Officer โ€” Home Care Division

Two offices. One in Brooklyn, NY. One in Clifton, NJ. Work out of whichever is closest to you.

You might be asking yourself โ€” who is launching a home care agency right now?

Fair question. The answer is someone who already has what most people spend years trying to build: existing Medicaid contracts, licensure, staff, and infrastructure already in place. This isn't a startup on paper. It's a division being launched from a foundation of real, proven success by a leadership team that knows how to build and scale healthcare organizations.

What they need now is the right operator to come in and run it.

The Role

This is a COO opportunity โ€” not a management role inside someone else's structure, but a true builder's seat. You will have direct access to the CEO and real authority to build the operational infrastructure of this division from day one.

What You'll Own

  • Day-to-day operations of the home care division across two offices
  • Relationships with home care agency partners โ€” building, maintaining, and managing them
  • Compliance oversight: DOH, Medicaid billing, documentation, EVV, and authorization management
  • Vetting and onboarding of staff serving as Certified Peer Specialists under the organization's Medicaid contract
  • Billing accuracy and session documentation across partner agencies
  • Operational systems, workflows, and policies that will scale as the program grows
  • Serving as the senior operational voice between the CEO and the agency partner network

What We're Looking For

  • A seasoned home care operator โ€” LHCSA, CDPAP, or home health background
  • Strong command of Medicaid billing, MLTC contracting, and DOH compliance
  • Experience managing multi-site or multi-program operations
  • Executive presence โ€” polished, direct, and comfortable at the leadership table
  • A compliance mindset paired with the ability to build structure in a fast-moving environment
  • Someone who has launched or grown programs before and knows what that actually takes

Compensation & Benefits

  • Competitive base salary commensurate with experience
  • Performance-based bonus tied to program milestones
  • Medical, dental, and vision insurance
  • Paid time off and holidays
  • 401(k) retirement plan
  • Flexibility to work out of the office closest to you

If you're a home care operator who's ready to build something first-of-its-kind with the backing of an organization that's already proven it knows how to succeed โ€” we'd love to connect.
