Job description

Patient Care Coordinator (Remote Telehealth)

Part Time | 1099 Contractor | Starting at $22/hour (based on experience)

Coastal Primary Care & Wellness is a fast-growing telehealth primary care and optimization clinic serving patients across multiple states.

We are hiring a Patient Care Coordinator to become the operational backbone of our patient experience.

This is not a traditional front desk role.

This role supports patient onboarding, scheduling, lab coordination, treatment workflow completion, and provider operations so our clinical team can stay focused on patient care.

Schedule

Starting schedule:

• Approximately 3 hours per day

• Monday through Friday

• Hours expected to grow as patient volume increases

Consistency and responsiveness during assigned hours are important.

What You’ll OwnPatient Scheduling & Intake

• Review appointment requests and schedule patients

• Create and manage patient portal access

• Verify intake forms and questionnaires are completed

• Collect and organize required documents

• Support patient onboarding and appointment readiness

Provider Readiness & Chart Preparation

• Review provider schedules daily

• Ensure charts are complete before visits

• Verify records, questionnaires, IDs, and supporting documents

• Flag incomplete charts for follow-up

Goal:

Providers begin visits prepared and focused on patient care.

Lab Coordination

• Monitor incoming lab results

• Coordinate scheduling of lab review visits

• Assist with processing provider-approved lab orders after payment

• Communicate next steps to patients

Important:

This role does not interpret labs or make clinical decisions.

Treatment & Order Coordination

• Support provider-approved treatment workflows

• Coordinate order processing through established systems

• Track confirmations and status updates

• Maintain accurate documentation and follow-through

Important:

Clinical decisions remain with licensed providers.

Patient Follow-Up & Workflow Management

• Follow up on incomplete tasks

• Monitor unpaid invoices and treatment delays

• Assist patients with completing onboarding requirements

• Help ensure patients continue moving through care efficiently

Communication & Operations

• Monitor inboxes and patient communication channels

• Respond to operational questions

• Escalate clinical questions appropriately

• Support provider workflow and operational efficiency
