This flexible position allows counselors to work remotely while maintaining close collaboration with our outpatient office team based in Rockford, Illinois. • Conduct thorough initial assessments and ongoing evaluations to tailor treatment plans according to the unique needs of each patient.
• Communicate effectively with supervisors, interdisciplinary team members, families, and collateral contacts to discuss client progress, treatment modifications, and care coordination.
• Create and maintain a tentative schedule that outlines appointments, home visits, and monitoring activities, ensuring effective time management and organization.
• Coordinate and conduct necessary home visits to fulfill requirements for in-home observations, assessments, and data collection related to client care.
• Guarantee proper and effective implementation of treatment plans by following up on sessions and modifying approaches as required.
• Engage in regular interdisciplinary team meetings and provide valuable insights regarding patient progress and program efficacy.
• Document detailed monthly and quarterly progress notes to track patient developments and adapt treatment plans accordingly.
• Organize and lead training sessions to educate team members on implementing treatment plans and maintaining accurate documentation.
• Provide ongoing support through follow-up interviews and additional home visits to ensure continuity of care for clients.
• Serve as the liaison between our organization and the wider mental health community, fostering partnerships that enhance service delivery and resource sharing.
• Assist in developing training materials and procedures that contribute to improving service quality and operational efficiency within our programs.

As a Licensed Clinical Professional Counselor with Riverwoods Academy, you will have the opportunity to work with diverse populations, including children, adolescents, and adults. Your understanding of the unique challenges faced by various demographics will be essential in developing comprehensive treatment plans that address each client’s individual needs. Our focus on trauma-informed care, family dynamics, and cultural competencies strengthens our approach to mental health services.

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.
• Active, unrestricted licensure as a Licensed Clinical Professional Counselor (LCPC) in the state of Illinois.
• Minimum of 2 years of counseling experience, ideally within a clinical setting.
• A specialization in areas such as family therapy, couples therapy, substance misuse, or trauma therapy is highly desirable.
• Demonstrated understanding of legal, ethical, and best practice standards in mental health.
• Exceptional interpersonal and communication skills, with the ability to establish rapport with a diverse client base.
• Proven ability to collaborate effectively within a multidisciplinary healthcare team.
• Proficiency in utilizing EMR software and other relevant technologies for documentation and reporting.

• Experience working with at-risk youth and families, particularly in an outpatient or community-based setting.
• Knowledge of evidence-based therapeutic modalities and their application in treatment settings.
• Familiarity with community resources, referral processes, and support services for mental health needs.
