Key Responsibilities
  • Conduct thorough initial assessments and ongoing evaluations to tailor treatment plans according to the unique needs of each patient.
  • Communicate effectively with supervisors, interdisciplinary team members, families, and collateral contacts to discuss client progress, treatment modifications, and care coordination.
  • Create and maintain a tentative schedule that outlines appointments, home visits, and monitoring activities, ensuring effective time management and organization.
  • Coordinate and conduct necessary home visits to fulfill requirements for in-home observations, assessments, and data collection related to client care.
  • Guarantee proper and effective implementation of treatment plans by following up on sessions and modifying approaches as required.
  • Engage in regular interdisciplinary team meetings and provide valuable insights regarding patient progress and program efficacy.
  • Document detailed monthly and quarterly progress notes to track patient developments and adapt treatment plans accordingly.
  • Organize and lead training sessions to educate team members on implementing treatment plans and maintaining accurate documentation.
  • Provide ongoing support through follow-up interviews and additional home visits to ensure continuity of care for clients.
  • Serve as the liaison between our organization and the wider mental health community, fostering partnerships that enhance service delivery and resource sharing.
  • Assist in developing training materials and procedures that contribute to improving service quality and operational efficiency within our programs.
Patient Population / Client Focus

As a Licensed Clinical Professional Counselor with Riverwoods Academy, you will have the opportunity to work with diverse populations, including children, adolescents, and adults.
