Salary Range: $85,000.00 To $105,000.00 Annually

The Registered Nurse (RN) provides nursing services to people with intellectual and developmental disabilities that reside in Intermediate Care Facilities (ICF) and/or Individual Residential Alternatives (IRA). The RN oversees the comprehensive health care for people living at Opengate and ensures that Direct Support Professionals (DSP) provide adequate health care in their assigned residences. The RN also functions as a member of the Interdisciplinary Team (IDT) assisting the people Opengate supports to obtain and maintain a level of wellness and independent functioning commensurate with their abilities.

This position is located at our Somers, NY facility.

Essential Job Functions

  • Administers treatment and/or medication as ordered by a physician.

  • Visits each residence according as scheduled to observe and report on the status and needs of the people living at Opengate.

  • Act as an advocate for people living at Opengate to ensure each receives appropriate health services to meet their needs.

  • Develop and implement personalized care plans based on each person's unique health needs.

  • Ensure vital signs, basic assessments, and medication administration are completed by AMAP staff and follow up to confirm accuracy.

  • Serve as a liaison between families, healthcare providers, and outside agencies to coordinate care and communicate health updates.

  • Communicate with Direct Support Professionals to evaluate the physical condition of people receiving support through Opengate.

  • Assist with the scheduling and tracking of appointments, consults, and documentation to ensure seamless coordination of services.

  • Complete ongoing assessments and documentation for in accordance with OPWDD regulation and agency policy and procedure.

  • Write Plan of Nursing Services (PONS), as well as regular notes for resident care documentation.

  • Reviews all health care provided to the people living at Opengate, including related materials, recording notes and findings in an electronic medical record system (i.e.; Precision Care).

  • Must attend an OPWDD Nurses' Orientation, if required to do so.

  • Receive and provide training in Diabetes Management, Medication administration OSHA, and other related required training, while maintaining current license.

  • Provide direction instruction to Opengate staff to pursue certification as Approved Medication Administration Personnel (AMAP), in content areas of medication administration, G-Tube management, and Diabetic management.

  • Maintain re-certification of Approved Medication Administration Personnel (AMAP) training for Direct Support Professionals (DSP) and other agency staff.

  • Maintains records/documentation associated with all instruction and certification, in accordance with Opengate policy, procedure and practice.

  • Regularly meet with the Director of Nursing Services.

  • Attend and complete all scheduled/required training and maintain current license.

  • Attend nursing training to remain current with evidence-based practice care as per OPWDD, NYS and agency guidelines.

  • Provide on-call nursing coverage, as needed.

  • Follow federal, state and local governmental regulatory guidelines pertaining to a safe, healthy, and clean work environment, reporting health and safety concerns to the supervisor.

  • Other duties as assigned.

Qualifications

  • Must be certified by NY State Education Department as a Registered Nurse.

  • 1+ years of experience working with individuals with intellectual and or developmental disabilities.

  • Proficient in basic computer skills.

  • Ability to report and record results of activities

  • Ability to work as a team member

  • Good oral and written communication skills

Salary: $85,000 - $105,000. Compensation will be commensurate with job qualifications and work experience
