Location: Colorado Springs, CO 80909

Position Overview

We are seeking a dedicated and experienced On Call Pharmacist specializing in Long Term Care to join our team. This role primarily supports our long-term care facilities by providing expert pharmaceutical care remotely. The position requires minimal in-office presence, allowing you to work mostly from home or another remote location. Shifts would be after pharmacy closes, starting at 8pm.

Key Responsibilities
  • Provide on-call pharmaceutical support for long term care facilities as needed.
  • Review and verify medication orders remotely to ensure accuracy and compliance.
  • Consult with healthcare providers and staff regarding medication management and safety.
  • Maintain closed door pharmacist responsibilities, ensuring patient confidentiality and regulatory compliance.
  • Respond promptly to urgent medication-related inquiries and issues during on-call hours.
  • Document interventions and communications accurately in the patient records system.
Qualifications
  • Licensed Pharmacist in the state of Colorado.
  • Experience in Long Term Care pharmacy practice preferred.
  • Ability to work independently and remotely with minimal supervision.
  • Strong communication skills for effective collaboration with healthcare teams.
  • Familiarity with pharmacy software and electronic medical records.
  • Commitment to maintaining patient confidentiality and professional ethics.
Work Environment
