The Physical Therapist Assistant (PTA) provides skilled physical therapy treatments and interventions under the direction and supervision of a licensed Physical Therapist. The PTA assists residents in achieving their highest practicable level of function, mobility, independence, and quality of life while maintaining compliance with facility policies, state regulations, and Medicare guidelines.

Pay Range: $28.15 - $37.00

What You Bring (Requirements)
  • Graduate of an accredited Physical Therapist Assistant program.
  • Current Physical Therapist Assistant license in good standing within the state of practice.
  • CPR certification required.
  • Previous experience in a Skilled Nursing Facility, post-acute care, long-term care, or rehabilitation setting preferred.
  • Knowledge of Medicare Part A, Medicare Part B, managed care, and skilled rehabilitation services preferred.

Essential Duties and Responsibilities
  • Provide physical therapy treatments as outlined in the established plan of care developed by the Physical Therapist.
  • Implement therapeutic exercises, gait training, balance activities, transfer training, strengthening programs, and functional mobility interventions.
  • Monitor resident responses to treatment and communicate progress, concerns, or changes in condition to the supervising Physical Therapist.
  • Accurately document daily treatment sessions, progress toward goals, and patient responses in accordance with regulatory and facility requirements.
  • Collaborate with nursing staff, physicians, rehabilitation team members, and interdisciplinary care teams to promote optimal resident outcomes.
  • Participate in care conferences and discharge planning as appropriate.
  • Educate residents, families, and caregivers regarding therapeutic exercises, safety precautions, and mobility recommendations.
  • Maintain treatment areas, equipment, and supplies in a safe and organized manner.
  • Adhere to all infection control, safety, HIPAA, and compliance standards.
  • Complete daily treatment notes, progress reporting, and other required documentation in a timely manner.
  • Ensure documentation accurately reflects skilled services provided and resident progress.
  • Experience with Net Health documentation software is preferred.
  • Maintain productivity standards while ensuring quality patient care and documentation compliance.
