Job description

Facility: LIFE PACE OLV
Shift: Shift 1
Status: Part Time w/Benefits FTE: 0.500000
Bargaining Unit: ACE Associates
Exempt from Overtime: Exempt: Yes
Work Schedule: Days with Call Weekend and Holiday Rotation
Hours: Alternating weekly M-W-F and Tu-Th 8 am to 4 pm
Summary:
The Registered Nurse, LIFE promotes and maintains health of participants in the community through teaching, counseling, and appropriate preventive and rehabilitative services. Responsible for assessment of health needs and contributing to the plan of care, initial implementation of nursing care plans, and providing nursing care, as well as periodic re-evaluation of individual and family nursing needs. Supervises medical assistants and LPNs in the clinic, to ensure regulatory compliance and quality while maintaining an optimal level of independence for participants. Additionally, provides nursing support to the LIFE primary care staff. Demonstrates knowledge and skills necessary to assess, plan care for, and provide service to frail elderly participants according to assigned responsibilities and LIFE Program standards.
Responsibilities:
EDUCATION
  • Graduate from an accredited school of nursing, college or university required
  • Licensure and current registration as a Registered Professional Nurse in the State of New York
  • BSN preferred
  • BLS certification preferred
  • Knowledge of rules and regulations governing Licensed Home Health Agencies preferred

EXPERIENCE
  • One year of experience working with the frail elderly required
  • Minimum one (1) year relevant experience required
  • Home Care experience preferred
  • Acute medical surgical experience preferred
  • Experience with patient assessment instruments preferred

KNOWLEDGE, SKILL AND ABILITY
  • Must possess a valid driver's license and have a car available for work use
  • Strong written & verbal communication skills
  • Computer skills required, i.e., software appropriate to position, word, excel
  • Ability to prioritize
  • Strong assessment skills required
  • Ability to multi task
  • Ability to work in a team environment
  • Adaptable to inside and outside environmental conditions
