A Brief Overview
The RN Case Manager (CM) will utilize their skills and expertise to assess the patient/family for necessary services within the continuum and provides appropriate resources and options for care. Collaborates with other members of the health care team on medically complex patients to monitor and resolve problems related to transitions of care including discharge; identifying opportunities to improve patient care and post discharge quality of service to help prevent readmissions; to identify inappropriate and/or untimely services and collaborate with other members of the health care team to monitor and resolve these problems; documents all activities per regulatory or hospital requirements. The delivery of professional nursing care at Stormont Vail Health is guided by Jean Watson's Theory of Human Caring and the theory of Shared governance, both of which are congruent with the mission, vision, and values of the organization.
Education Qualifications

  • Bachelor's Degree Bachelor's of Science in Nursing (BSN) Preferred


Experience Qualifications

  • 2 years Acute care, broad based nursing experience in a hospital setting. Preferred

  • Quality management/Joint Commission, discharge planning and/or case management experience is equivalent to acute care nursing experience Preferred

  • Experience conducting clinical assessments, setting up home care services, general discharge planning and/or case management . Preferred

Licenses and Certifications

  • Registered Nurse - KSBN A multistate license with the ability to practice within the State of Kansas is also accepted. Required


What you will do

  • Performs admission assessments and documents findings accurately and in a timely manner.

  • Facilitates coordination of care for ambulatory services i.e. infusion center, wound care, DME, and PT/OT.

  • Facilitates discharge planning in conjunction with the health care team for patients transitioning to the next level of care i.e. discharging home with home health, to nursing facilities, acute rehab, LTACH, and Hospice.

  • Communicates with all members of the health care team informing them of any concerns or recommendations regarding d/c needs or plans.

  • Provides resources to patients/families to educate them about different levels of care and help them to understand their treatment options, and provides required documentation of Important Message from Medicare prior to discharge.

  • Initiates hospital issued notice of noncompliance (denial of service) letters if/when necessary.

  • Keeps supervisor informed of all quality and risk issues discovered through appropriate channels.

  • Identifies area for process improvement and participates in related activities.

  • Appropriately delegates tasks and duties in the direction and coordination of health care team members, patient care and department activities in accordance with the Kansas State Nurse Practice Act.

  • Participates in a regular rotating schedule of on-call responsibilities. When on-call, individual must be able to physically report to SVH within 30-45 minutes if the need arises.
