Position Summary
The Crisis Case Manager - Youth Crisis and Recovery Center is designed to address the needs of persons served ages 5-17 who are at risk of entering or are returning home from high intensity services such as inpatient settings, YCRC services, or psychiatric residential treatment facilities. The Crisis Case Manager will provide medically necessary services at the direction of a qualified mental health provider skilled in the treatment of mental disorders to assist the served individual and their families in reducing the effects of the mental and emotional impairment for which institutional level of care has been indicated. This is to help maintain placement of the youth in the home and thereby help maintain stability in the community. The Crisis Case Manager provides goal-directed support and solution-focused interventions intended to achieve identified goals and objectives as set forth in the served individual's treatment plan with emphasis on maintaining safety and stability for person served with the help of their support systems. Crisis Case Management is with the person served, guardians/caregivers, and other treatment team and community members. The majority of Crisis Case Management contacts occur face-to-face and in community locations where the individual served and family members live, work, attend school, and/or socialize. The services will be specifically for youth and their families with the goal to enhance independent and interpersonal functioning such that the individual is integrated into and/or maintained within the home and community so that institutionalization is not as likely or frequent.
ESSENTIAL FUNCTIONS
- Provide direct services to youth and their families who are enrolled in ICRP, ITCOD, and for up to four weeks post discharge to assist with transition to daily living.
- Provide direct service for youth and their families post discharge of PRTF for up to 30 days
- Provide direct services including community psychiatric supportive treatment (CPST) based on the philosophies and principles of the strengths model.
- Provide support to the youth/family with evidenced-based interventions for emotional and behavioral management with the goal of assisting the youth/family with developing and implementing social, interpersonal, self-care, daily living and independent living skills to restore stability and support functional gains.
- Assist the youth, family members, and other collaterals as appropriate with identifying strengths and needs, resources, natural supports by providing Parent Support, Peer Support, and Parent Peer Support services.
- Assist in clarifying goals and objectives that use personal and family strengths, resources, and natural supports to reduce the risk of institutional level of care.
- Facilitate wellness management and recovery strategies.
- Model skills for and provide consultation to youth, families, and fellow team members.
- Provide crisis services, including mobile responses, as needed for the served individual/family.
- Provide parent support and/or parent peer support services to assist in developing a positive strengths based environment in the home.
- Provide targeted case management for coordination of multiple services for youth and families across different departments and organizations.
- Act as an advocate for children/youth and families. Collaborate with community, academic, and legal programs to increase functioning for the youth and their family.
- Assist in tracking progress of the person served and their family by utilizing objective, valid, and reliable measures at the direction of the YCRC Clinical Manager or other qualified mental health professional.
- Attend team meetings and actively participate in the review of served individuals and their families' progress and services.
- Facilitate discharge and transition back home by coordinating with person served, treatment team members, parents/caregivers, and other identified collaterals.
- Complete required trainings.
- Complete required background checks.
- Complete documentation (progress notes, treatment plan reviews, etc.) in accordance to agency standards. YCRC Case Managers are also expected to stay informed of agency developments by reading memorandums and other agency related written communication.
- Respect and maintain confidentiality for the person served. Report any breech to supervisor and Risk Manager.
- Participate in all team activities and consult with team members as an equal professional.
- Provide cross training to other team members in recovery principles and strategies.
- Act as an ambassador of Astra Mental Health and Recovery in the community setting.
- Support and actively promote the vision and mission of Astra Mental Health and Recovery and the philosophy of the strengths perspective.
- Other duties as assigned by respective team manager or supervisors.
POSITION QUALIFICATIONS
Education/Experience:
- Associates degree in a human services field, or 3+ years of experience in a human services field.
- Must have 1+ years of experience working with children, adolescents, and/or families.
- Experience working with youth identified with severe emotional disturbance (SED) preferred.
- Must successfully pass the KBI, SRS and all Central Registry background investigations.
- Valid driver's license, current auto insurance, a good driving record, and a reliable vehicle are required

Other Physical Requirements:
The YCRC Case Manager must have sufficient ambulatory capabilities to work with children, adolescents, young adults and their families in the families' homes and community-based setting. Position may require the ability to use physical holding (MAB) with children and adolescents exhibiting out of control behaviors.
WORK ENVIRONMENT
Office and community-based working environment; requires flexible schedule; frequent interruptions occur; expected to deal with individuals and families in crisis; risk of exposure to verbally and physically aggressive youth; and exposure to low socioeconomic homes and neighborhoods.
