Job description

                                                                                                              Quality Workflow Nurse


   

General Summary/Responsibilities:
Responsible for HCHB workflow/Oasis review/SHP alerts
Responsible for reviewing the clinical information received on Oasis documentation.  In addition, the Quality Workflow Nurse will work closely with the clinical departments to ensure proper HCHB/Oasis documentation are being followed and appropriate Oasis assessments are being completed.  The Quality Workflow Nurse
will also be involved in quality audits in relation to HCHB documentation and Oasis/Outcomes

DIMENSIONS:
    Works with HCHB workflow/Oasis review/SHP alerts
    Lead the quality focused agency initiatives 
    Participates in Oasis review and SHP alerts process under PPS guidelines
    Participates in initial and ongoing education and training for Oasis documentation for clinical staff.
    Maintains knowledge and competency in the use of computer information systems.
    Pursues ongoing education and training on Oasis guidelines for home health care.
    Facilitates interdepartmental communications and cooperation to ensure appropriate coding and Oasis completion.
    Participates in audit activities for compliance issues.

    Acts as a role model for customer service activities.
    Monitors and coordinates with supervisors and management issues regarding trends with Oasis assessments and SHP alerts.
    On-Call weekend and holiday back up for HCHB RN
 

Qualifications 
Education

Minimum Level of Education Required: Associate degree
Additional requirements:
    Type of degree: Associates or Bachelors
    Area of study or major: Nursing
 

Licensure/Certification/Registration

    Current Ohio RN Licensure in good standing.

Experience

Minimum Level of Experience Required: 1 - 3  years of job-related experience
Prior job title or occupational experience: Certified Oasis Specialist-Clinical (COS-C) required within 1     year of hire date.
Preferred experience: OASIS/POC review experience preferred
 

Knowledge/Skills
    Strong organizational skills required.
    Understanding of HCHB Workflow and Home Health Oasis assessment guidelines required.
    Possess sound judgment and ability to communicate with individuals from all levels of the organization and work well under pressure.
    Computer experience required.
    Possesses and demonstrates strong communication and problem solving skills.
    Positive customer service skills required.
    Works well with deadlines.
