Job Description:
Position Summary
Under the leadership of the director and assistant director of Hospice of the Central Coast, the Case Manager is a Registered Nurse responsible for the smooth operations of the hospice office, fielding informational calls that require an RN, speaking with MD's, taking telephone orders, assisting nurses in the field, speaking and instructing families and patients on hospice. The Hospice Case Manager possesses the skills and knowledge to competently provide appropriate nursing care on the phone or in person. The Case Manager is also responsible to make adjustments in visit schedules, schedule the nurse practitioner, and document and complete re-certifications. The Case Manager will have the IDG complete and up to date for scheduled meetings. The Case Manager will notify Assistant Director or Director of any problems.
Experience
Three years experience as a nurse is required. A minimum of two years home hospice experience within the last five years is required. Spanish speaking preferred.
Education
Bachelor's degree in nursing preferred.
Must pass orientation and initial competency assessment prior to independent assignment of patients requiring the applicable skill, and must complete and pass all annual competencies.
Licensure/Certifications
State of California RN license required. American Heart Association Healthcare Provider BLS required. Hospice certification preferred. Must possess a valid California driver's license.
This position requires driving. In addition to a valid CA driver's license, the following are required: Automobile insurance as required by the State of California with an acceptable driving record of three points or less, participation in the DMV pull-notice program, and an automobile available for daily work.

The hours employees work determine when a shift differential is paid.
Hourly Evening Shift Differential: $5.50
