GENERAL SUMMARY:

Provides personal care, nursing assistance to residents; assists in daily living activities, provides resident documentation and evaluation.

RESPONSIBILITIES:

  • Takes and records vital signs: temperature, pulse, blood pressure, and respirations as required; recognizes abnormalities.
  • Participates in development of residents’ plan of care and follows current plan of care; promptly reports a change in conditions of residents.
  • Demonstrates comprehensive physical assessment skills for initial and

follow up appraisal of resident status; analyzes initial and on-going assessments in the development of the plan of care.

  • Provides & assists residents in bathing/showering for thorough cleanliness and hygiene; assists residents with grooming and hygiene (i.e. shaving, oral hygiene, cleaning, trimming finger/toenails) in a safe and thorough manner on a daily basis.
  • Assists with feeding as necessary; assess for choking and responds appropriately.
  • Assists with toileting in a safe & thorough manner per residents’ plan of care.
  • Assists with balancing/standing activities; bed mobility, techniques and ambulation and the use of assistive devices.
  • Assists residents in dressing with concern for privacy while providing care.
  • Makes beds, straightens rooms, and keeps resident care areas in an organized manner.
  • Provides safe resident transfers and lifting techniques involving bed, wheelchair, chair and/or shower chair.
  • Maintains inventory of medical equipment/devices for residents (wheelchair, assistive devices); ensures equipment is in good condition and notifies maintenance for repair.
  • Promotes independence in resident care as appropriate; demonstrates awareness of safety for both residents and the individuals providing care.
  • Assists residents with appropriate range of motion or other exercises as directed by nurse or therapist and ambulates residents safely according to his/her endurance levels/care plan.
  • Applies medical or adaptive devices as ordered by the appropriate supervising nurse.
  • Makes rounds to ensure residents' safety, comfort, medical attention every hour or as required.
  • Collects specimens as directed; weighs resident as required.
  • Delivers nourishments in a timely manner/ assisting as necessary.
  • Performs and documents weekly/monthly weight monitoring for designated residents; reports significant weight gain/loss to Licensed Nurse.
  • Knowledge of universal precautions and proper use of gloves, gowns, masks, eye protection and linen handling.
  • Follows documentation procedures as required by Title XXII and OBRA rules and regulations; uses accurate medical terminology.
  • Completes a screening, contracture assessment, and physical functioning assessment for residents referred to the RNA program.
  • Documents progress/decline in ADL/ROM/transfer abilities, completes weekly summaries, and communicates progression with therapists/nurses in accordance with the established policies and procedures.
  • High school graduation or GED required. 
  • Valid California Certified Nurse Assistant certification.
  • 6-12 months experience in providing personal care in a long-term care facility preferred
  • Current first aid certification
