The Care Manager RN provides professional, comprehensive, patient-centered care management services for at-risk patients in an acute care environment. This role focuses on coordinating care across the continuum to ensure quality outcomes, appropriate resource utilization, and safe discharge planning.
Key responsibilities include:

โ€ข Coordination of care across multidisciplinary teams

โ€ข Discharge planning and transitions of care
โ€ข Documentation of interventions and outcomes
โ€ข Regulatory and compliance oversight

Qualifications:
