A compassionate, dedicated RN Case Manager who makes patients feel at home—whether they’re in their own home, an assisted living facility, or a skilled nursing home. If you’re a hospice or home health Jedi with a valid GA RN license, driver’s license, and at least one year of experience, we want to meet you.
