Job Description
The Bluebonnet Trails Community Services (BTCS) Forensic Assertive Community Treatment Team Intensive Case Manager (FACT ICM) is responsible for providing outstanding person-centered intensive case management services as part of the Forensic Services Team. The target populations are adults who are currently involved in or at risk of involvement in the criminal justice system and have a diagnosed severe and persistent mental health disorder.
This position reports directly to the TCOOMMI/FACT Director and requires the development and maintenance of effective, professional working relationships with legal stakeholders and other community service providers with focus on integration of assertive community treatment teams, mental health deputies, and crisis services. This is a Monday through Friday, non-exempt, field-based position with flexible hours - no on-call or nightshift.
The FACT ICM regularly meets with individuals in their homes, community, probation/parole, court, and at Bluebonnet Trails' clinics to provide case management and psychosocial rehabilitative services. Services are intended to stabilize mental health symptoms, target substance use treatment needs, reduce risk of recidivism, increase the individual's awareness of and participation with community and natural supports, develop self-advocacy, and assist with their ability to participate in independent behavioral health services. A high level of detailed electronic documentation is required and adherence to company and state guidelines is expected. A minimum of 55% of hours worked are required to be a combination of billable and non-billable participant services.
The person in this position must possess a passion and aptitude for working with individuals who have chronic and severe mental health needs and are involved in the criminal justice system. Knowledge of forensic assertive community treatment programs, psychotherapeutic medications, intensive case management functions, substance use treatment, crisis services, community resources, and evidence-based practices in the mental health and criminal justice systems is preferred. This position necessitates innovative thinking, patience, resiliency, skills in advocacy, service coordination, mental health assessment and evaluation, collaborative person-centered recovery planning and dual-diagnosis treatment. The person in this position will effectively work independently and on a team, prioritize needs during crisis situations, engage in regular meetings and training opportunities, balance self-care, be a delight to work with, and perform other duties as assigned with a positive attitude.

Hourly Salary Range: $21.46 - $28.71
Differential Detail:
  • Up to $1.00/hour - Previous experience in a similar role
  • Up to $4.00/hour - Tenure with BTCS
  • $1.00/hr. - LCSW-A, LPC- A, Advanced Licensure
  • $0.75/hr. - Bilingual
  • $0.50/hr. - LCDC

City Seguin, TX County Guadalupe County Credentials Minimum Qualifications:
A bachelor's degree from an accredited college or university with a minimum number of hours that is equivalent to a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education/intervention.
Education and experience in behavioral healthcare, intensive community treatment, and the criminal justice system.
Must have a valid Texas Driver's License, meet Bluebonnet Trails Community Services driving requirements, and maintain approved driving status throughout employment.
*If degree is not in the Human Services field, a minimum of 24 college credits in the Human Services field are required.
Preferred Qualifications:
Previous experience working in Forensic Assertive Community Treatment Programs, behavioral health, criminal justice, and/or case management.
Advanced degree in Human Services field.
QMHP certification.
Licensed Practitioner (LCDC, LCSW, LMFT, LPC).
