Job Title: Registered Nurse - Case Management

Location: Apple Valley, CA

Contract Duration: 13 Weeks

Schedule:
Day Shift | 5x8 Hours
8:00 AM - 4:30 PM

Orientation: 40 Hours (Non-Billable)

Position Overview:
We are seeking an experienced Registered Nurse Case Manager for an Acute Care Facility in Apple Valley, CA. The RN Case Manager will coordinate patient care plans, support discharge planning initiatives, and collaborate closely with interdisciplinary healthcare teams to ensure quality patient outcomes and continuity of care.

Responsibilities:
Coordinate and manage patient care plans throughout hospitalization
Perform utilization review and discharge planning activities
Collaborate with physicians, nurses, social workers, and ancillary departments
Facilitate safe and timely patient discharges
Assess patient needs and coordinate post-acute services and resources
Maintain accurate documentation and case management records
Advocate for patients and ensure quality, cost-effective care delivery
Participate in interdisciplinary rounds and care coordination meetings

Requirements:
Active California RN License required
Minimum 2 years of Acute Care Case Management and/or Discharge Planning experience required
Hospital acute care experience required
Behavioral Health experience alone will not be accepted
Experienced traveler required
First-time travelers will not be considered
