The Case Manager plays a vital role in delivering client-centered services that support patients across the continuum of care. This position partners with patients and their families to coordinate healthcare and connect them with appropriate home- and community-based services.
You will ensure timely access to medically appropriate care while promoting continuity, quality outcomes, and a positive patient experience.

Required Qualifications
  • Active, unrestricted Registered Nurse (RN) license in the State of Florida (must be maintained)
  • 3+ years of clinical experience
  • 1+ year of case management experience preferred

  • Strong computer proficiency, including Excel
  • Experience with Electronic Medical Records (EMR) systems (Athena experience a plus)
  • Excellent communication, organization, and problem-solving skills
