Social Work Manager

Role Summary
Our client is seeking a compassionate and experienced Social Work Manager to lead their Social Work team and guide the delivery of high-quality social services and behavioral health programs. This is a leadership opportunity to shape care delivery, support patients in meaningful ways, and foster collaboration across care teams and community partners. If you’re a motivated leader with a passion for patient-centered care, this role offers the chance to make a lasting difference.

Setting: Out Patient Clinic

Population: Geriatric population

Duties

  • Lead and mentor the Social Work team, including Supervisors, Social Workers, Behavioral Health Specialists, and Assistants—providing hiring, training, supervision, and coaching.

  • Oversee the quality and effectiveness of social work assessments and team participation.

  • Ensure delivery of all social work interventions aligned with patient care plans and responsive to urgent needs.

  • Maintain compliance across social work functions, including documentation, assessments, care planning, reviews, and patient rights and grievance processes.

  • Collaborate with Behavioral Health Specialists to design and oversee support groups, individual counseling, family education, and community partnerships.

  • Support patients and families through end-of-life care planning and coordination.

  • Conduct home visits as needed to support patients’ ongoing care.

Required Qualifications

  • Master’s degree in Social Work (MSW).

  • Active state driver’s license with clean DMV record.

  • Proof of recent vehicle insurance.

  • Minimum 1 year of experience working with a frail or elderly population.

  • Excellent clinical, organizational, and communication skills with patients, families, and team members.

Desired Qualifications

  • Social work licensure (preferred).

  • At least 2 years of supervisory or leadership experience in social work.
