Telehealth Physician (Remote | Full-Time | No On-Call) Monday- Friday 9am-6pm

What Youโ€™ll Do
  • Provide routine telehealth visits with Medicare patients (E/M and Chronic Care Management)
  • Create and manage personalized care plans for ongoing conditions
  • Collaborate with our integrated care team (nurse case managers, NPs, CHWs)
  • Ensure thorough documentation in our EHR and compliance with quality measures
  • Engage patients in preventive care to reduce avoidable complications

Requirements

What Weโ€™re Looking For
  • MD or DO, Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics
  • 1+ year of physician experience in family medicine or internal medicine
  • Active medical license โ€” multi-state licensure a plus
  • Experience in preventive care, senior health, or chronic condition management is a plus
  • Comfortable practicing via telehealth and using EHR systems is a plus
  • Experience providing preventive virtual care is a plus
