Position: Residential Mentor

Reports to: Program Supervisor

Supervises: No positions

Primary Hours: Set based on needs of individuals in service Hours Vary

Required Qualifications:

Current valid Arizona Driver's License (to be a driver)
Clean driving record, no more than 2 moving violations in a 3-year period (to be a driver)
Twenty-one (21) years of age or older (to be a driver)
Able to communicate effectively with individuals in service and team members using both verbal and written skills
Able to use computer proficiently

General Responsibilities:

  1. Provide active direct physical assistance, supervision and behavioral support to individuals served in Agency programs
  2. Demonstrate safe workplace behavior as defined by Agency workplace safety plan
  3. Be honest

Specific Duties and Responsibilities:

Individuals Served
  1. Provide opportunities to acquire, use and maintain skills which enable individuals served to live as independently as possible
  2. Maintain an on-going knowledge of the individuals served
  3. Maintain the privacy of individuals served
  4. Serve as a positive role model for individuals served
  5. Assist in the development of written objectives and skill plans
  6. Carry out skill development plans, recommendations, agreements, assignments, and routines as required by the Agency
  7. Assist individuals served in planning and participating in social, recreational, and educational activities at home and in the community
  8. Actively assist individuals served with problem solving
  9. Assist individuals served with planning, purchasing and/or preparation of foodstuffs to ensure nutritional intake within the context of the budget
  10. Assist with and/or manage funds for individuals served as assigned
  11. Assist individuals served with personal hygiene, and health & safety issues that affect them.

Lifting, moving, positioning, and transporting individuals served; (Hoyer Lifts, Wheelchairs, gate belt etc.) Must be able to lift 50 pounds to help provide support and safety for individuals supported.

  1. Obtain, administer, and maintain medications for individuals served
  2. Provide transportation for individuals served (if hired in a driving position)
  3. Attend, document and provide feedback for meetings and/or medical appointments for individuals served
  4. Purchase household and hygiene supplies as required
B. Operations/Equipment
  1. Provide routine and on-going housekeeping and home maintenance of interior and exterior of the residence to provide a neat, clean, healthy, and safe environment
  2. Conduct and participate in evacuation and emergency plan drills and document the results
  3. Maintain and utilize all Agency equipment and appliances in a manner which will promote their longevity (i.e. observe maintenance schedules, repair as needed, follow manufacturer's directions for use, etc.)
C. Professional/Operational
  1. Responsible for meeting all times assigned for scheduled duty
  2. Requests and notifies Supervisor of leave times in accordance with Agency procedure
  3. Interact and communicate with individuals served, their team members, other Agency personnel, and members of the community in an appropriate and professional manner
  4. Document the progress/status of individuals served and maintains files and records in accordance with Agency standards using Agency record keeping systems
  5. Attend required Agency and Agency-related meetings as directed
  6. Comply with all Agency Policies and Procedures as well as all applicable regulations and laws
  7. Acquire and maintain all certifications and training required by the Agency in a timely manner
  8. Complete special tasks and other duties as assigned
