The Mobile Nurse Practitioner provides comprehensive primary and/or specialty healthcare services to patients in facility-based settings including Board & Care Homes (B&C), Assisted Living Facilities (ALF), and Skilled Nursing Facilities (SNF). This is a mobile role responsible for delivering patient-centered care directly in the field across Los Angeles and Orange County, with an immediate focus on the San Fernando Valley and West Los Angeles areas.

  • Ability to travel throughout assigned service areas in Los Angeles and Orange County.
  • Strong clinical assessment, diagnostic, and care coordination skills.
