Job Description

Direct Support Professional provides direct support and assistance for the consumer with or without the caregiver present, in or out of the consumer’s residence, to achieve and maintain the outcome of increased independence, productivity, enhanced social functioning, and inclusion in the community, as outlined in their service plan.
Community-based support is delivered one on one with a consumer. The DSP will work under the direction of the agency Director. Maintains compliance with agency policies and procedures and follows state and federal regulations.
Duties:

Assist consumer in development of community living skills Assist consumer in preparation of meals, maintaining a household, toileting, dressing, grooming, and personal maintenance and assist in assuring there are opportunities for the consumer within the home to participate in their own lives and to become part of the community possible.
This may include, but is not limited to:
  • Plan and participate in recreational activities of the participant’s choosing
  • Identify and pursue outside interests, hobbies, involvements, and events
  • Develop, foster, and maintain relationships with family and friends
  • Foster independence and maximize involvement in all daily living skills through goal-directed, creative, imaginative efforts to promote and protect self-worth, respect ,and dignity
  • Provide supportive assistance in all aspects of direct residential care including but not limited to:
  • Administer medication consistently and timely, following the medication administration procedures if applicable
  • Grooming
  • Hygiene and toileting
  • Meals (following menus and all nutritional and dietary guidelines)
  • Cleaning and laundry
  • Mobility
  • Budgeting/Spending
  • Individual therapeutic needs, as necessary
  • All personal health and safety needs
Provide support and maintain continued involvement with consumers, especially with regard to:

  • Health, welfare, and happiness
  • Relationships, Interests, capacities, and talents
  • Social skills and activities
  • Goals, dreams, and aspirations
  • Preferences and choices
  • Maintaining the confidentiality of consumers and protected information always
And any other assigned job duties

Physical Requirements:
The employee must be physically capable of transferring, assisting with transfers, and lifting persons who require physical assistance.
The employee must be able to reach and bend as they assist participants with physical activities.
Experience/Education/License Requirements

  • Bi-lingual, must speak (English and Spanish)
  • High school diploma or equivalent
  • Valid New Jersey Driver’s License (driving record not to exceed 5 points). If transporting consumers’ proof of automobile insurance is needed
  • Must be 21 years or older
  • Must clear any State/Federal criminal background checks, the Central Registry and CARI database
  • CPR/FA certification
  • Must be proficient in English (read, write & comprehend)
  • Complete all training for the position within established timelines
OTHER GENERAL REQUIREMENTS:

Ability and willingness to support individuals with challenging behaviors, ability to adapt to environmental conditions in private residences
Maintain safety and cleanliness of any personal vehicle that transports consumers Display professionalism always
Provide at least two (2) verifiable job references.
