The Licensed Clinical Social Worker will be responsible for a variety of critical tasks, including but not limited to:

• Conducting comprehensive psychosocial assessments to evaluate the emotional and social factors affecting recovery, utilizing interviews and thorough medical record reviews.
• Providing individualized counseling and emotional support to patients and their families, addressing adjustment issues related to injury or illness.
• Collaborating with the rehabilitation team to develop tailored treatment plans that incorporate patients' psychosocial needs and goals.
• Facilitating access to community resources such as support groups, vocational rehabilitation services, and housing assistance.
• Participating in discharge planning to ensure a smooth transition to home or other care settings, helping to mitigate any post-rehabilitation challenges.

You will be working with a diverse demographic of patients who are navigating the challenges of physical and emotional recovery from conditions such as stroke, spinal cord injuries, orthopedic surgeries, and neurological disorders. Understanding and addressing the unique psychosocial barriers faced by this patient population is essential for their successful rehabilitation.

To be considered for this role, candidates must possess the following:

• A Master’s degree in Social Work from an accredited program.
• Active licensure as a Licensed Clinical Social Worker (LCSW) in New York State.
• Strong understanding of psychosocial dynamics, adjustment issues, grief, and coping mechanisms.
• Exceptional communication and interpersonal skills to engage effectively with patients, families, and team members.

While not mandatory, the following attributes will enhance a candidate's application:
