Position Summary
The Family Nurse Practitioner (FNP) provides comprehensive primary care services to diverse patient populations, including adult and geriatric patients, in a collaborative and patient-centered environment. This role focuses on preventive care, chronic disease management, patient education, and delivering high-quality clinical care to support positive patient outcomes.
Responsibilities
  • Provide primary care services including preventive, acute, and chronic care management
  • Assess, diagnose, and treat a wide range of medical conditions
  • Manage chronic diseases and pain management treatment plans
  • Educate patients on wellness, disease prevention, and healthy lifestyle practices
  • Collaborate with physicians and multidisciplinary healthcare teams to ensure quality patient care

Qualifications
