Case Manager – Skilled Nursing
Pay: $20.00 to $24.00 per hour
Champion Care is seeking a compassionate, organized, and detail-oriented Case Manager to support our skilled nursing community. In this role, you will coordinate resident care, collaborate with the interdisciplinary team, and help ensure each resident receives the services and support needed to achieve the best possible outcomes.

Responsibilities
Coordinate resident care plans with nursing, therapy, social services, physicians, and other healthcare professionals.
Complete initial and ongoing resident assessments to identify needs and support appropriate care planning.
Monitor resident progress and communicate changes to the interdisciplinary team.
Help ensure smooth admissions, discharges, and transitions of care.
Work with residents and families to provide education, support, and resources.
Maintain accurate and timely documentation.
Assist with insurance authorizations, Medicare and Medicaid requirements, and payer communication as needed.
Participate in care conferences and interdisciplinary team meetings.
Support quality improvement initiatives and promote positive resident outcomes.
Qualifications
Previous experience in case management, care coordination, admissions, social services, nursing, therapy, or another healthcare-related role preferred.
Experience in a skilled nursing, rehabilitation, long term care, hospital, or healthcare setting is strongly preferred.
Working knowledge of Medicare, Medicaid, managed care, and insurance authorization processes is a plus.
Excellent communication, organization, and problem-solving skills.
Ability to manage multiple priorities in a fast-paced environment.
Strong computer and documentation skills.
A passion for providing outstanding resident care and customer service.
