Position Overview

Premier Health and Wellness Center is actively seeking a dedicated and skilled Physical Therapist (PT) to join our rehabilitation team on a full-time basis in Elizabethport, New Jersey. This position offers the opportunity to work closely with patients, implementing tailored treatment plans to enhance mobility, alleviate pain, and improve the overall quality of life for those in our care.

Work Environment

This role is primarily onsite, providing home care services, where you will engage with patients in their homes, ensuring a comfortable and familiar environment for their rehabilitation exercises. As part of your day-to-day responsibilities, you may need to adjust therapy sessions based on the specific conditions and requirements of each patient, demonstrating flexibility and adaptation to various settings. You will be working in collaboration with an interdisciplinary team, including Occupational Therapists and Speech-Language Pathologists, to ensure comprehensive care strategies are in place.

Key Responsibilities
  • Conduct thorough assessments to develop individualized treatment plans aimed at achieving specific patient goals.
  • Guide patients through therapeutic exercises, stretches, and other activities designed to enhance physical function and reduce pain.
  • Educate patients on proper techniques for body mechanics and home exercise programs to facilitate their recovery.
  • Administer and monitor modalities such as electrical stimulation and heat therapy as part of the treatment regimen.
  • Document patient progress meticulously, tracking improvements in areas such as range of motion and functional abilities.
  • Prepare the treatment environment, ensuring all necessary tools and supplies are ready and organized for effective therapy sessions.
  • Collaborate with team members to optimize treatment protocols and share insights on patient progress.
