About the Role
As a Care Coordinator, you'll independently assess needs, coordinate services, and advocate for adults with opioid use disorders receiving Medication Assisted Treatment (MAT). You'll help ensure clients receive comprehensive, coordinated care while maintaining continuity of services.
Key Responsibilities
