Join Chapa-De Indian Health as our SUD RN Case Manager in Grass Valley! In this role, you'll combine your nursing expertise, compassion, and care coordination skills to support patients with substance use disorders (SUDs), including those participating in our Medication Assisted Treatment (MAT) Program. You'll collaborate with providers, behavioral health teams, and community partners to help patients access the care and resources they need.

Hourly Pay: $52.65-$60.00

What You'll Do

  • Provide nurse case management and care coordination for patients with SUDs, including patients receiving MAT.
  • Assess patient needs using ASAM criteria, evaluate suitability for MAT, and coordinate behavioral health referrals and higher levels of care.
  • Support patients throughout the induction, stabilization, and maintenance phases of MAT treatment.
  • Build strong partnerships with community, county, state, and Indian Health Service agencies to strengthen continuity of care and patient resources.
  • Support key SUD initiatives, including SBIRT, Smoking Cessation, MAT grants, program data collection, reporting, and compliance requirements.
