Job description

Join a dedicated Assertive Community Treatment (ACT) Team focused on supporting individuals with severe and persistent mental illness who have not benefited from traditional clinic services. This role involves outreach, enrollment, and providing comprehensive mental health and social support to help clients maintain housing and achieve fulfilling community lives. As an ACT Team clinician, you will deliver wellness self-management counseling, integrated dual disorder treatment, and assist clients in improving daily living skills, especially those with a history of poor engagement in conventional mental health care. You will collaborate closely with other treatment teams to provide intensive, community-based services aimed at keeping clients supported and stable.
Essential Functions:
- Participate in daily team meetings to assign cases and review responsibilities
- Conduct comprehensive assessments for new clients and determine appropriate ACT services
- Use trauma-focused, person-centered, evidence-based practices to address mental health symptoms and improve well-being
- Develop crisis prevention plans and treatment goals in partnership with clients
- Coordinate care with community partners, families, physicians, and other providers
- Support clients in achieving treatment goals, medication education, crisis management, and daily living activities
- Monitor client safety as needed and complete ongoing diagnostic evaluations
- Assist with discharge planning and transitions to community or higher levels of care
- Foster a person-centered, outcome-oriented program culture dedicated to quality improvement
- Provide culturally competent services and complete all required documentation and training timely
- Work a flexible schedule including some evenings, one weekend day, and on-call rotations to support clients during crises
Education and Experience:
- Master's Degree in Social Work, Psychology, or Marriage and Family Therapy required
- Preferred: 3-5 years experience working with severe mental illness or serious emotional disturbance populations
- Experience with diverse and high-risk client populations and integrated care approaches preferred
Licensure and Certifications:
- Licensed at Masters Level Clinician by the appropriate state board required
- Qualified Mental Health Professional (QMHP) training required
- Valid Driver's License required
- Must obtain temporary Masters Addiction Counselor licensure within 6 months and full licensure within 12 months of employment
- Preferred: Clinical Level Licensure and Clinical Addictions Counselor License
