As a remote professional, you will engage with the team through virtual meetings and maintain essential documentation using our health information systems.

As a Licensed Clinical Social Worker, your daily tasks will include:

• Conducting in-depth psychosocial assessments through interviews, observations, and medical record reviews to identify the emotional and social factors influencing patient health.
• Offering individual and family counseling designed to help cope with the challenges associated with end-of-life issues, grief, and emotional distress.
• Collaborating with interdisciplinary teams to formulate comprehensive, individualized care plans tailored to each patient's psychosocial requirements.
• Connecting patients and families with vital community resources, such as bereavement support, legal aid, and financial assistance services.
• Actively participating in patient discharge planning, ensuring smooth transitions between care settings, and providing follow-up care as necessary.
• Organizing support groups or educational workshops for families, enhancing their understanding of grief processes and caregiving challenges.

In this role, you will work primarily with terminally ill patients and their families, assisting them through the emotional and practical challenges they face. Your efforts will aim to enhance the quality of life for both patients and their caregivers, ensuring that people receive the emotional support they require during one of the most challenging times in their lives. You will facilitate resources that allow for a dignified and fulfilling end-of-life experience.

The ideal candidate will possess:

• A Master’s degree in Social Work from an accredited program.
• Active licensure as a Licensed Clinical Social Worker (LCSW) in the State of Kansas.
• A strong understanding of end-of-life care modalities, bereavement, grief counseling, and coping strategies.
• Excellent interpersonal and communication skills, including the ability to engage empathetically with patients and families.

While not mandatory, preference will be given to candidates with:

• Prior experience in hospice, palliative care, or a rehabilitation environment.
• Demonstrated knowledge of community resources relevant to hospice care.
• Experience utilizing electronic health records (EHR) for documentation and care planning.
