Join the VNA Personal Navigator Program as a Personal Navigator & Outreach Coordinator and make a meaningful impact in the lives of individuals and families throughout Montgomery County.In this role, you'll connect with community members, conduct benefits eligibility screenings, guide clients through applications and renewals, advocate on their behalf, and connect them with additional community resources. You'll also build relationships with local organizations and represent the program at community events and outreach activities.We're looking for a compassionate, organized, and confident professional with strong communication and case management skills who enjoys working independently while building meaningful connections in the community.If you're looking for a rewarding position where your work directly helps people access the support they deserve, we'd love to hear from you!RESPONSIBILITIES AND ESSENTIAL DUTIES:(Responsibilities include but are not limited to)Provide screening, intake, advocacy, enrollment, acquisition, renewal, and/or referral assistance to any Montgomery County or Phoenixville area residents who seek assistance through the program and are eligible for any public benefit program (SNAP, Medical Assistance, CHIP, LIHEAP, amongst others). Provide enrollment assistance through the PENNIE Health Insurance Marketplace, COMPASS or Medicare.gov for uninsured children and adults to access subsidized, low-cost, and free health insurance programs. This includes completing applications, gathering required documents, and providing impartial, unbiased information about coverage and renewal options. Distribute outreach materials to community members and partners to build public awareness. Attend, represent, and/or present at community events to promote the agency and program. Actively participate in local networking opportunities and collaboratives. Strong interpersonal and customer service skills; the ability to work comfortably with clients, volunteers, and staff of diverse backgrounds; and a high level of comfort with vulnerable and underserved populations.The ability to recognize cultural, language, and learning differences and be able to assist individuals in an affirming and culturally appropriate manner. Must be a team player and able to cultivate good working relationships both internallyas part of integrated services and externally with staff at government offices where applications are filed, for troubleshooting and advocacy with those offices. Must be a collaborative problem solver, effective communicator, and embrace a diverse team environment Complete all necessary annual certifications to assist with certain health care enrollment platforms. Including participation in webinars, conference calls, and in-person trainings for continued professional development regarding the Marketplace, Medicare, and other benefitsMust follow all federal regulations and requirements put forth by the Centers for Medicare and Medicaid Services (CMS), including, but not limited to, privacy and security standardsMaintains confidentiality in dealing with VNA-Community Services, Inc., its customers, vendors, clients, and patients. Complies with HIPAA regulations and agency policies related to HIPAA compliance Maintains active use and personal proficiency with computer and software programs specific to office communication and agency program management –Microsoft applications, G-Suite, and other applications essential to specific job requirementsTrack all data needed for case management, tracking, reporting, and grants, including but not limited to the number of clients served by location, benefits applied for, benefits received, status, renewal dates, and relevant case notesAccepts other assignments as requested or directed by VNA for work within the scope of the employee's general job description, education and/or licensure. Education/Qualifications: Education: An undergraduate degree in a relevant field such as social work, psychology, public health, social services, or a related discipline is preferred. Case management experience required. Master's degree preferred. Communication Skills: Excellent verbal and written communication skills, with the ability to engage with diverse community members and present information concisely and effectively.Empathy: A genuine empathy for community needs and a compassionate approach to addressing concerns and building trust.Problem-Solving: Strong problem-solving skills and the ability to find creative solutions to challenges vulnerable populations face.  Cultural Competence: Sensitivity to cultural diversity and an understanding of how cultural factors may impact dynamics and interactions of varying cultural backgrounds. Organizational Skills: Effective organizational and time management skills. Ability to work and meet deadlines independently, as well as with a team. Networking: Strong networking abilities to connect with community leaders, organizations, and resources.Computer Skills: Proficiency in basic computer applications and the ability to use digital tools for communication and data collection.Local Knowledge: Familiarity with Montgomery County human services and local resources. As well as community demographics and the social and economic challenges it faces.Social Justice: Possesses a strong interest in social justice and has experience doing advocacy work for underserved populationsPublic Benefits Experience: Experience in providing direct service or enrollment of individuals into benefits access programs. Awareness of public health insurance programs, such as PENNIE, CHIP, Medical Assistance, and Medicare. Knowledge of eligibility and qualifications for government programs and healthcare terminology. Awareness of other government programs that help to alleviate poverty, such as SNAP, TANF, CHIP, rent rebates, etc Bilingual Spanish with experience with immigration statuses preferred.
