We are hiring a Physical Therapist (PT) for a Home Health position providing skilled care to patients in their homes in the Clarkston and surrounding areas.

Responsibilities
  • Deliver skilled, patient‑centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you.
  • Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence.
  • Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home.
  • Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
  • Work independently, but never alone.
  • Travel to patient homes within assigned territory.
Qualifications
  • Active Physical Therapist license in the state you work.
  • One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care.
  • Current driver’s license and ability to spend approximately 20% of your day driving to/from patient locations.
  • Successful completion of a drug screen prior to employment, which includes a medical and recreational marijuana policy.
