Overview

Good Samaritan University Hospital, a member of Catholic Health, is committed to delivering patient-centered, high-quality care to the communities we serve. We are seeking an experienced Social Work Supervisor to lead our social work and care coordination team with a strong focus on advancing complex, social work-driven cases and optimizing patient progression throughout the continuum of care.

Job Details The Social Work Supervisor provides clinical and administrative oversight of hospital-based social workers and care coordinators. This leader ensures effective case progression, timely discharge planning, psychosocial intervention, and seamless transitions of care. The role partners closely with nursing, physicians, care coordination, and interdisciplinary teams to remove barriers, reduce length of stay, and support safe discharge outcomes.This is a hands-on leadership role ideal for a clinician who is passionate about operational excellence, staff development, and high-quality social work practice in an acute care setting. Key ResponsibilitiesLeadership & Supervision 
  • Supervise, mentor, and support social work and care coordination staff
  • Conduct case reviews and provide clinical guidance on complex, high-risk cases
  • Lead daily escalation meeting and weekly case progression/long length of stay meetings focused on care coordination and social work-driven barriers
  • Monitor productivity, documentation quality, and regulatory compliance
  • Participate in hiring, onboarding, and performance evaluations
 Clinical & Case Progression Oversight 
  • Ensure timely psychosocial assessments
  • Oversee complex discharge planning including SNF placement, behavioral health transfers, homelessness, guardianship, APS/CPS involvement, and community linkage 
  • Address barriers to discharge and escalate appropriately to leadership
  • Collaborate with utilization management to support appropriate level of care
  • Promote patient advocacy and culturally competent practice
  Operational & Quality Improvement 
  • Track and improve metrics related to length of stay, avoidable days, and readmissions
  • Identify workflow gaps and implement process improvements
  • Ensure compliance with NYS regulations, CMS guidelines, and hospital policy
Qualifications
  • Master's Degree in Social Work (MSW) from an accredited program
  • Current New York State LMSW required; LCSW preferred
  • Minimum 3-5 years hospital social work experience
  • Prior supervisory or leadership experience preferred
  • Strong knowledge of discharge planning, community resources, and regulatory requirements
  • Demonstrated ability to manage complex, fast-paced clinical environments
Posted Salary RangeUSD $43.92 - USD $60.78 /Hr.
