Job description

Are you looking for an opportunity to work in a caring and community focused environment? At Covenant Community Care, we are a faith based non-profit, Federally Qualified Health Center serving the communities of Detroit in our clinics that offer integrated medical, dental and counseling healthcare services. We have an immediate opening for a Full-time Behavioral Health Therapist.
GENERAL DESCRIPTION: The Behavioral Health Provider will provide integrated and outpatient counseling services for pediatric to adult patients within a primary care setting. Care is provided with an emphasis on integrating screening and brief interventions, as well as follow-up/outpatient care as appropriate.
SPECIFIC DUTIES:
  • Work as part of an interdisciplinary team to help in recognizing, treating, and managing mental health, substance use and psychosocial issues as appropriate.
  • Support patients in their health goals, including, treatment compliance, sleep, coping with persistent health issues, or other.
  • Conduct mental health and substance use screening; provides referrals as clinically appropriate. Referrals may include psychiatry, housing, basic needs or other.
  • Provide psychoeducation to patients and their families concerning mental health and substance use issues.
  • Provide assessments, treatment plans and evidence based therapeutic treatment for patients in individual, family or group settings. These visits may be in an integrated or outpatient setting.
  • Document the patients' progress and diagnostic information in the treatment chart.
  • Communicate patient progress with the care team as appropriate.
  • Maintain confidentiality of patient health information in accordance with HIPAA, 42 CFR part 2 and other applicable regulations.
  • Cover other behavioral health provider's patients as needed
  • Maintain compliance with MI Licensing, including completing continuing education as required.
  • Uphold the Mission of Covenant "To show and share the love of God as seen in the Good News of Jesus Christ by providing integrated, affordable and quality health care to those who need it most."
  • Other duties as assigned.

  • Accepting Limited Licensed Master's Social Worker (LLMSW) or Licensed Master's Social Worker (LMSW) candidates
  • Two years experience

  • Dependable, organized and the ability to efficiently prioritize work
