Job description

Behavioral Health Home Health Nurse

Silver Lining is a patient-centered home health, home care, and behavioral health provider dedicated to delivering compassionate, evidence-based care to individuals with behavioral health needs in the comfort of their homes. We partner with patients, families, and multidisciplinary teams to improve mental and physical health outcomes, enhance quality of life, and support community independence.

Key Responsibilities
  • Provide comprehensive in-home nursing care to patients with behavioral health conditions, including medication management, symptom monitoring, and safety planning.
  • Conduct mental status exams, risk assessments, and crisis intervention as needed; collaborate with physicians, psychiatrists, therapists, social workers, and case managers.
  • Develop and implement individualized care plans in collaboration with the patient, family, and care team; adjust plans based on changing needs and goals.
  • Administer medications, injections, and treatments per physician orders; monitor for adverse effects and interactions.
  • Provide psychoeducation to patients and families on mental health conditions, medications, adherence, and coping strategies.
  • Support recovery-oriented goals, including adherence to treatment, housing stability, and social determinants of health.
  • Coordinate transitions of care, arrange appointments, and ensure timely follow-up; advocate for community resources and supports.
  • Document thoroughly in electronic health records (EHR); ensure compliance with HIPAA and agency policies.
  • Participate in case conferences, quality improvement initiatives, and staff education.
  • Provide crisis intervention and de-escalation as needed; know when to escalate to higher levels of care.
  • Promote patient safety, fall prevention, and health literacy in the home environment.
Qualifications
  • Registered Nurse (RN) license in Illinois with appropriate scope and responsibilities as allowed by state regulations and employer policy.
  • BSN preferred; additional certification in Behavioral Health, Psychiatric Mental Health Nursing (RN-BC or PMHNP) a plus.
  • Minimum three years of nursing experience; background in behavioral health, psychiatric-mental health, addiction services, or community/home health preferred.
  • Experience with psychosocial assessments, medication management, and crisis intervention.
  • Strong interpersonal and communication skills; ability to build rapport with patients and families from diverse backgrounds.
  • Knowledge of community resources, social determinants of health, and care coordination processes.
  • Proficiency with EHR systems, charting, and documentation; strong organizational skills.
  • Current CPR/First Aid certification; valid driver's license and reliable transportation; willingness to travel within assigned service area.
  • Willingness to participate in on-call rotation or weekend coverage if required.
