We are currently seeking a dedicated and passionate Care Manager to join our team during our 3rd shift and weekends.

**Job Summary:**
The Care Manager will provide exceptional support and care to our clients during the overnight hours and weekends. This role involves coordinating and delivering clinical and non-clinical services to meet the individual needs of clients, ensuring their safety, comfort, and overall satisfaction. The Care Manager will also collaborate with families, healthcare professionals, and community resources to promote positive outcomes for those in our care.

**Key Responsibilities:**
- **Client Care:** Assess and identify clients’ needs through direct observation and interaction, providing personalized care and support.
- **Coordination:** Collaborate with healthcare providers, family members, and interdisciplinary teams to create and implement individualized care plans.
- **Monitoring:** Regularly monitor clients’ health status, documenting changes and communicating effectively with relevant parties.
- **Support:** Offer emotional and psychological support to clients and their families, addressing any concerns or questions they may have.
- **Crisis Management:** Respond to emergency situations with professionalism and care, employing conflict resolution and de-escalation techniques as necessary.

**Qualifications:**
