Job description

Here at Community House Mental Health Agency, we strive to provide a supportive family atmosphere in Seattle and King County for people with severe and persistent mental illness.ย Each person who walks through our door has a unique story, and we believe that everyone deserves to feel welcomed and valued.

Within our agency sits our Diversion Services department. Those on our Diversion Services team work with the King County and the WA State Department of Social and Health Services to provide the necessary care and treatment to those in the legal system. These services include legal intervention, competency restoration, outreach programs, and peer services.

The Diversion Services department has a fantastic opening for a Residential Case Manager!ย 

The purpose of this role is to provide coverage assistance across all residential sites in the Diversion Services department. This role willย work as part of a team to provideย behavioral health services, skills training, and community support in a residential setting within their assigned diversion program. This position is responsible for providingย treatment and care coordination based on resident needs and the treatment plan, progressย notes, and collaborative reviews for the assigned caseload, as well as support to allย residents served as part of the care team.

SCHEDULE

Thursday-Monday, 10:00AM-6:00PM (Full-Time, 40 hours/week).ย This position does require travel to different CHMHA facilities.

RESPONSIBILITIES

  • Serve asย secondaryย Case Managerย and primary on-site contactย forย residents atย assignedย location,ย including advocatingย forย and coordinatingย residentย needs and services; assistingย withย Service Plans, Crisis Plans,ย andย otherย documentationย andย providing consistent contacts,ย skills training, and care coordinationย to work towards goalsย 
  • Coordinate with relevant internal and external departments to coordinateย residentย referrals, ensureย timelyย intake into program, andย review ofย individualized plansย 
  • Collaborate with care team to ensureย all residentsย access needed treatment, support, case management, and appointments, including transportation to external providersย 
  • Ensureย all interactionsย representย theย five principles of trauma-informed care, including safety, choice, trustworthiness, collaboration, and empowermentย 
  • Coordinate access to medications forย allย clientsย through trained staffย ensuringย theย sevenย rights of medication use are followed: rightย person, rightย medication, right time, right dose, right route,ย right reason (including refusal), and right documentationย according to agency provided medication trainingย 
  • Activelyย participateย in Shift Change meetings and complete associated documentationย 
  • Complete daily documentation in electronic medical record on resident interactions, medications dispensed when assigned, and other required documentation, ensuring an interaction or observation andย subsequentย documentation are completed for all residents across the teamย 
  • Practice good financial stewardship of agency and contract resources, including how time is being spent, supplies are used, property is beingย maintained, etc.ย 
  • Monitor spending ofย Petty Cashย and other agency money to ensure accountability andย appropriate useย 
  • Meetย identifiedย key performance indicators and metrics for the position asย identifiedย for the program, division, and agencyย 
  • Participateย in discharge planning and written summaries of treatment as needed forย residentย referral and releases of informationย 
  • Assistย with and support on-site cleaning, including skills training with residents toย maintainย personal and community spaces, laundry, and preparationย for regular pest treatmentsย 
  • Provide immediate crisis intervention and resolution as neededย 
  • Alwaysย maintain residentย and staff confidentialityย 

The Diversion Residentialย Case Managerย works with care teamย to ensureย high standardsย of care are provided within theย program, including but not limited to:ย ย 

  • Providing appropriate levels ofย treatment,ย skills training,ย and assistance are provided toย residentsย inย managing mental health symptomsย andย developing skillsย and insightย to manage symptoms, includingย Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) per their service plan and individual needs, such as assistance in hygiene needs, cleaning, finances, transportation, etc.ย 
  • Partnering as a team to deliver current programming and fill vacant position dutiesย 
  • Working collaboratively with larger outpatient team of providers to supportย residentย in actively working towards treatment goalsย 
  • Coordinatingย care, such as scheduling, attending, and/or arranging transportation for appointments and other case management needsย 
  • Maintaining andย documenting in residentย chartsย and program trackersย following agency, county, state, and federal guidelines
  • Providingย and receivingย input as part ofย residentโ€™sย care team on service plan and progress, and collaborating with other providers such as primary care provider,ย housing staff for entering/maintaining/exiting housing, etc.
  • Facilitatingย regular group and individual activities, encounters, and specializedย residentย events, such as outings and holiday gatheringsย 
  • Performing other duties as assigned

BASIC QUALIFICATIONS

  1. Apply for Agency Affiliated Counselor credential with Washington Department ofย Health within 30 days of hire, if not otherwise licensed, and obtain and maintainย registration and/or licensure

  2. Successfully pass criminal background check before hire date

  3. Obtain/maintain valid First Aid/CPR card or certificate within 30 days of hire

  4. Hold a valid license for 1 year with clean driving record to use agency vehicles

  5. Have legible handwriting and ability to do simple mathematics

  6. Able to navigate and comprehend charts in electronic medical record

  7. Able to work independently and within a team

  8. Able to communicate information and ideas so others will understand. Able toย exchange accurate information in these situations.

  9. Able to perform duties outlined above within reasonable accommodation

PREFERRED QUALIFICATIONS

  1. Prior experience with mental health or diversion services population

  2. Bachelorโ€™s degree in social sciences or related field
