Respiratory Therapist – COPD Transitional NavigatorRole Summary

We are seeking a dynamic and compassionate Respiratory Therapist COPD Transitional Navigator to lead patients through a seamless transition from hospital care to home wellness. By delivering heart-centered education, conducting clinical assessments, and coordinating safe discharges, you will directly reduce readmissions and elevate the quality of life for individuals across all age groups. Duties & Responsibilities
  • Empowering Patient Education: Deliver comprehensive, engaging education to patients and caregivers utilizing diverse tools like lung models, handouts, and videos. Topics include normal lung anatomy, medication action plans (what, when, how, and why), recognizing exacerbations, smoking cessation, oxygen use, and COPD-specific nutrition.

  • Transitional Navigation & Care: Act as a critical liaison between physicians, case managers, and discharge planners to ensure an optimal length of stay, a safe discharge plan, and direct referrals to pulmonary rehab and pulmonary function testing.

  • Proactive Follow-Up: Conduct meaningful follow-up care via telephone post-discharge to answer questions, ensure medication compliance, and confirm essential appointments with primary care providers.

  • Comprehensive Respiratory Care: Perform all aspects of respiratory therapy with clinical excellence for both inpatients and outpatients across a diverse population, including neonatal, pediatric, adolescent, adult, and geriatric patients.

  • Collaborative Support: Partner with the broader healthcare team to champion organizational standards, participate in continuing education, and contribute to a safe, positive, and efficient care environment.

  • Other duties as assigned.

Required Qualifications
  • Education: Graduate of an AMA-approved program for Respiratory Therapy.

  • Licensure & Credentials: * Current and valid New York State (NYS) Respiratory Care License.

    • Credentialed as a Registered Respiratory Therapist (RRT), or tracking to obtain it within 6 months of hire.

  • Certifications: Current CPR/BLS and ACLS certifications are required.

  • Clinical Expertise: Expert knowledge of COPD, clinical respiratory assessments, and pulmonary diseases.

  • Core Competencies: Exceptional interpersonal and verbal communication skills; outstanding time management, judgment, and multitasking abilities; and a strict commitment to confidentiality.

  • Orientation: Successful completion of the facility orientation program upon hire.

Desired Qualifications
  • Specialized Certifications: Credentialed as a COPD Educator via the American Association for Respiratory Care (AARC) is highly preferred.

  • Wellness Expertise: Certified as a Tobacco Cessation Specialist is a strong plus.

  • Work Type: Full-Time, Direct Hire (Varied shifts, including weekends to support continuous patient care navigation).

  • Work Setting: On-site acute and outpatient care settings.
