Job description

Director - Case Management

We are hiring a Director of Case Management to oversee and manage all aspects of case management operations within a 400+ bed acute care hospital. This role is responsible for ensuring effective care coordination, discharge planning, and optimal utilization of healthcare resources while maintaining compliance with organizational and regulatory standards.

Key Responsibilities:

  • Lead and manage day-to-day case management operations across the hospital

  • Oversee staffing, workflows, and departmental performance

  • Ensure compliance with policies, regulatory standards, and documentation requirements

  • Drive care coordination and discharge planning processes

  • Collaborate with executive leadership (CFO, CMO, CNO) and interdisciplinary teams

  • Monitor performance metrics and implement process improvement initiatives

  • Reduce length of stay and improve patient outcomes

  • Lead, mentor, and develop a high-performing team

Qualifications:

  • Bachelor's degree (required)

  • Master's degree in Nursing, Healthcare Administration, or Business Administration (preferred)

  • Active licensure required: RN, LCSW, or LMSW

  • 3+ years of acute care hospital case management experience

  • 2+ years of leadership experience
