b. Complete all required assessments with individuals and their families

c. Provide collaborative documentation

d. Keep all credentials and certificates up to date

DESCRIPTION OF WORK DUTIES AND RESPONSIBILITIES:

Employees are expected to perform their work in a competent and

efficient manner.

Duties Include:

Staff will provide Outpatient assessment services (BHA, ARA, and SPD) via telehealth. Staff will provide quality collaborative documentation, maintain all required trainings and certifications, attend all staffing and agency or other required trainings.

Staff will attend staff meetings as required. Staff will be a team player with all programs and staff in the agency.

Staff will maintain productivity as required and complete all other duties as assigned.
