LVN MDS Coordinator

Lakewest Rehabilitation and Skilled Care is looking for an experienced LVN MDS Coordinator to join its dynamic nursing team. The ideal candidate for the LVN MDS Coordinator position will be an LVN, possess strong organizational skills, be responsible for notifying and coordinating with the interdisciplinary teams for MDS assessment completion in accordance with State and Federal regulations. Candidates for the LVN MDS Coordinator position must also have at least 1 year of MDS and PASRR experience.
