Licensed Practical Nurse (LPN)

Weekend Doubles: 7:00am-3:00pm, 3:00pm-11:00pm

OR 3:00pm-11:00pm, 11:00pm-7:00am

We are seeking a Licensed Practical Nurse (LPN) to provide direct nursing care and support daily clinical operations within our skilled nursing facility. This role ensures residents receive quality care in accordance with established standards and regulations.

Responsibilities

  • Administer medications and treatments as ordered

  • Monitor, record, and report changes in resident condition

  • Document assessments and care provided in the electronic medical record

  • Assist residents with daily needs as appropriate

  • Communicate effectively with residents, families, and interdisciplinary team members

  • Maintain infection control, safety, and confidentiality standards

  • Support the nursing team to ensure consistent, highโ€‘quality care delivery

Qualifications

  • Current LPN license in the state of employment

  • Graduate of an accredited practical nursing program
