This includes participating in care plan meetings, sharing patient updates, and coordinating services such as therapy or specialty consultations. Effective communication is essential, as you'll often serve as a liaison between patients, families, and various care providers. Building strong relationships within the team helps streamline workflows and improves patient outcomes.

What are the key skills and qualifications needed to thrive as an LPN Care Manager, and why are they important?

Familiarity with electronic health records (EHRs), care planning software, and relevant healthcare regulations is typically required.
