Position Summary

As a Part-Time LPN at Luna Del Valle Hospice, you will deliver direct patient care in the home setting, under the supervision of a Registered Nurse (RN). This role is essential to ensuring high-quality, compassionate care that respects patient dignity and supports families during a vulnerable time.

Responsibilities

Administer medications and treatments per Hospice policies and clinical standards

Perform baseline nursing skills with clinical competency and in accordance with state practice acts

Monitor patient condition and promptly report any changes, needs, or concerns to the supervising RN

Assist in developing and updating patient-centered Plans of Care, including measurable goals and interventions

Maintain accurate and timely documentation of nursing interventions, observations, and education provided

Provide emotional support to patients and families, always upholding patient rights and dignity

Communicate effectively with physicians, interdisciplinary team members, patients, and families

Participate in team conferences and contribute to collaborative care planning

Promote teamwork by supporting and assisting fellow staff members when needed

What You Bring

Graduate of an accredited Licensed Vocational/Practical Nursing Program

Current and valid LPN license in the State of New Mexico

CPR certification and a valid driver's license with auto insurance

At least 1 year of nursing experience (Hospice experience preferred)

Strong organizational, time management, and communication skills

A cooperative, compassionate approach to patient care

Ability to handle emotional demands with grace and professionalism

Basic math skills for medication and treatment administration

Additional Requirements

Maintains patient confidentiality and adheres to HIPAA and privacy policies

Participates in continuing education and in-service trainings

Keeps current with health care regulations and standards

May be required to participate in occasional on-call rotation
