For 80 years, we have been one of upstate New York's largest providers of programs and services for individuals who have disabilities. Many of the innovative programs and vital services that we offer are not available elsewhere.

What We Are Looking For:Camp LPN Health Care Coordinator

Where:Camp Clover Patch located in Glenville, NY at our Clover Patch Camp.

Dates of Camp: 5/30/2026 - 7/17/2026

Orientation-5/30/2026

This is a 30 hour/week position.

Responsibilities:

Residential Program Specific Duties:

  • Will provide nursing coverage depending on needs of programs as assigned by the Camp Health Director.
  • Will work with healthcare team in meeting the health care needs of the individuals that we support.
  • Will collaborate with Registered Nurse in providing the needed in-service training to the staff.
  • Will write and maintain nursing progress notes as needed.
  • Will review the MAR's, scripts and Consumer Specific Formularies to ensure accuracy when required.
  • Will collaborate with residential teams in providing oversight and guidance in health care management.
  • Will keep nursing administration abreast of any health concerns related to the individuals we support.

Camp Program Specific Duties:

  • Will review all charts for Cloverpatch Campers.
  • Will prepare all nursing documents for Cloverpatch Camp (Progress Notes, health data, MARS).
  • Will provide direct nursing care at Cloverpatch Camp
  • Will communicate with the RN (Camp Health Director) with any medical/nursing concerns for campers.
  • Will contact each camper's caregiver(s) prior to the scheduled camp week to reconcile medication orders.
  • Will work under the statutes of the Nurse Practice Act with regards to delegation of nursing functions providing the necessary training and documentation.
  • Will provide additional services as deemed necessary by the Residential Nursing Administration team and the Camp Health Director

Requirements:

  • High School diploma or equivalent
  • Specialized training, license, certification, skills: Current NYS LPN License
  • At least 2 years of job related experience, specifically in the field of Nursing.
  • At least 2 years of job related experience preferred specifically working with individuals with developmental disabilities.
