Licensed Mental Health Professional (LPC, LCSW, LMHC, LMFT) 

In-Person Behavioral Health Services in Skilled Nursing Facilities 

Full-Time | Part-Time | Flexible Scheduling

We are seeking fully licensed mental health professionals to provide in-person psychotherapy and behavioral health services within Skilled Nursing Facilities (SNFs) and Assisted Living communities. This position offers an opportunity to develop meaningful relationships with patients while working independently and enjoying the support of an established multidisciplinary team. Providers enjoy the flexibility and autonomy of private practice without the burden of managing a business.

Responsibilities 

  • Provide in-person psychotherapy and behavioral health services within Skilled Nursing Facilities and Assisted Living communities.  

  • Conduct diagnostic assessments and develop treatment plans.  

  • Treat depression, anxiety, adjustment disorders, grief, trauma, and other behavioral health concerns common among geriatric populations.  

  • Maintain timely and accurate clinical documentation.  

  • Collaborate with interdisciplinary care teams to improve patient outcomes.  

  • Build therapeutic relationships with residents and their families.  

Qualifications 

Required 

  • Active and unrestricted Pennsylvania license as one of the following:  

  • LPC

  • LCSW  

  • LMHC  

  • LMFT  

  • Master's degree or doctoral degree in an applicable discipline.  

  • Ability to provide compassionate, patient-centered care.  

  • Strong communication and documentation skills.  

  • Comfortable providing services in nursing home and long-term care settings.  

Preferred 

  • Experience working with geriatric populations.  

  • Experience in Skilled Nursing Facilities, rehabilitation centers, long-term care, or assisted living settings.  

  • Interest in interdisciplinary and integrated healthcare.  

Important Licensing Requirement 

Only Fully Licensed Providers Will Be Considered.
