Faith-based family health care center seeking a full-time (40 hours per week) Behavioral Health Therapist to join our growing behavioral health team! The Behavioral Health Therapist acts as an advocate and helps clients gain access to community-based resources and counsels patients on a 1:1 basis regarding their personal issues, such as mental illness, trauma, addiction and abuse. Complete a psychosocial assessment that includes social, economic, cultural, age-related factors, history of child, elder or domestic abuse, prior psychiatric or substance abuse treatment, family and social support systems.Coordinates and plans programs and activities to meet the patient's social and emotional needs.Collaborates with other members of ELFHCC's behavioral health team to provide co-management activities for patients who require additional services.Complete a psychosocial assessment that includes social, economic, cultural, age-related factors, history of child, elder or domestic abuse, prior psychiatric or substance abuse treatment, family and social support systems.Develops and implements a comprehensive, interdisciplinary plan of care which includes patient preferences.
