Job description

RN Long Term Case Manager

Immediate need for a talented RN Long Term Case Manager. This is a 03 months contract with possible extension or conversion opportunity and member facing role in Harlingen, TX (Onsite).

Job Diva ID: 26-23846 Pay Range: $40.20 - $43.60 /hr. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities
  • Assess the needs of complex and high-acuity members and develop individualized care plans.
  • Coordinate medical, behavioral, and community-based services across the continuum of care.
  • Collaborate with physicians, specialists, caregivers, and community organizations to ensure members receive appropriate care.
  • Monitor member progress, evaluate changes in condition, and update care plans as needed.
  • Conduct home and/or community visits (typically once per month or as required) to assess member needs.
  • Review referrals and intake assessments to determine appropriate care management strategies.
  • Educate members and caregivers on disease management, available benefits, healthcare resources, and community services.
  • Coordinate service authorizations for long-term care services such as transportation, meals, housing, and activities of daily living.
  • Maintain accurate documentation and ensure compliance with state, federal, and organizational guidelines.
  • Identify care gaps, monitor quality outcomes, and recommend improvements to enhance member care.
  • Serve as a liaison and advocate between members, families, healthcare providers, and community resources.
  • Support onboarding and mentoring of new clinical staff as needed.
Key Requirements and Technology Experience:
  • Key skills: RN, Long term care, LTC, Home visits, Case Management
  • Graduate of an accredited School of Nursing.
  • Active Registered Nurse (RN) license in the state of practice or Compact State Licensure.
  • Bachelor's degree with related clinical experience preferred.
  • 4–6 years of healthcare or care management experience preferred (not required).
  • Ability to travel locally for member visits.
